In an effort to assist other offices with getting patients the assistance needed in an efficient and timely manner, We have have identified
specific information that is needed for us to properly assess patients prior to their appointment. These items are as follows:
? Patient demographics sheet. (Name, DOB, SSN, Address, Phone#, Insurance…..)
? Physician Referral.
? All Chart Notes & Records referencing the condition that the patient is being referred for.
? All records/reports
from other modalities of treatment that have been tried.
? Complete Medications list.
? Insurance Referral. (When required by Insurance plan.)
We fully realize that all patients will not have everything on the above
list of documents. However, it is necessary that as much of the requested information be provided as quickly as possible so
that the patient can be scheduled and receive the required treatment.
Our goal is to provide patients with quality care and service, your assistance with providing this information and aiding us with the treatment and care of patients is greatly appreciated.
